Step 4: Enter your full name, email address and phone number. From the pull down menu, locate and select "St. Lawrence the Martyr Church, Chester"
Step 5: A verification mail should arrive to your email verifying your email
address and connecting it with our parish subscription. Click the sign in link and you should immediately have access to FORMED.
